I've been living with IBS for some years now, and have been managing it reasonably well. Apart from watching the kinds of food I eat, I take gastric flora capsules (the ones available from the chemist), and omeprazole once a day.
The only differences I have noticed between my body's responses during fistplay and those who do not have IBS, is that:
the walls of my rectum can be more sensitive to lubes that have glycerin or glycol as an ingredient, and it burns in a way I DON'T like!
if I have eaten something I shouldn't have the day before a play, the pre-play cleaning out process takes much longer; and even with a good wash-out, it isn't always successful; and
after an IBS episode, my elasticity sometimes reduces, and while I might have taken 2 hands before a partcular IBS episode, once the worst is over, it's an effort just getting one hand in. And an uncomfortable effort, too.
But oddly enough, a good arseplay session can be just what I need to get my innards back in a regular routine!
I thought I'd post this to see if any other IBSers have made any observations and maybe have hints/tips to share?
